1. Fill this form completely and submit it
2. An ELITE sales rep will help follow up once the submitted info is received in
3. Elite Screens requires an Official Non-Cancelable Purchase Order from reseller/distributor
4. Elite Screens requires 100% payment upfront once the order is confirmed. For orders placed through an Elite Screens dealer, payment terms may apply.
5. Estimated time of delivery for custom screen is up to 150 business days from the date of PO/full payment
